Strep throat is a bacterial infection caused by group A Streptococcus. It inflames the throat and tonsils and can make swallowing painful.

Because strep and viral sore throats can feel similar, testing helps confirm whether antibiotics are appropriate. Symptoms alone cannot reliably diagnose strep.

Strep throat symptoms often come on quickly. Common signs include:

• Sore throat, often severe
• Pain when swallowing
• Fever
• Red, swollen tonsils — sometimes with white patches or streaks
• Tiny red spots on the roof of the mouth
• Swollen lymph nodes in the front of the neck
• Headache, stomach pain, or nausea (more common in children)
• Fatigue or feeling unwell

Strep throat spreads through respiratory droplets when someone who is infected coughs, sneezes, or shares close contact.

It is different from most sore throats caused by viruses such as the common cold or flu. Viral illnesses more often include cough, runny nose, or hoarseness — though overlap is common, which is why testing matters.

Close contact in households, schools, and workplaces can increase exposure, especially during colder months when respiratory illnesses circulate more frequently.

A healthcare provider reviews your symptoms and examines your throat and neck. If strep is suspected, a rapid strep test may be performed during your visit using a throat swab.

Rapid tests can provide results during the same appointment in many cases. If results are negative but strep is still strongly suspected, a throat culture may be recommended — culture results can take longer but may detect infections a rapid test misses.

Testing helps avoid unnecessary antibiotics for viral illnesses and supports appropriate treatment when strep is confirmed.

When strep is confirmed, antibiotics are typically prescribed. They can shorten how long you feel sick, reduce spread to others, and lower the risk of certain complications when taken as directed.

Most people begin to feel better within a few days of starting antibiotics, but it is important to finish the full course unless your provider advises otherwise.

Contagious period: People with strep are generally most contagious when symptomatic. After starting appropriate antibiotics, contagiousness usually decreases — often within about 24 hours — though your provider may give guidance based on your specific case, symptoms, and setting (such as school or work).
